  Effective Multidisciplinary Treatment
 

Mr. X, a case of TB Spine had been a patient of BYL Nair Hospital where he had undergone surgery of the spine. He was brought to Health Harmony Project on 26/3/03. He came on a stretcher, had two pressure sores, and no bowel or bladder control at the time of admission. He could not get up or sit on his own & was so spastic that his legs could not be taken apart.
He was given accomodation in HHEP on 26/3/03 for his rehabilitation. He was 23 yrs old, was educated upto S.S.C
and had previously worked as a cook in a big hotel at Nashik..


During his stay at the HHEP he was given Occupational Therapy Physiotherapy, Counselling care and hep of his pressure sores. As per the usual policy, his mother was asked to stay with him and help in his care and therapy . However it was soon evident that because of over protection of his mother, he was not co-operating in the treatment & was insisting that mother should do everything for him. He was not willing to plan for his future.

The social worker counselled both the mother & the patient. Later, as a part of therapy, the mother was prepared to leave the patient alone & go home for two days. The staff & other Orthopaedically disabled patients gave him only marginal help. This experience gave the patient & mother confidence which was re-reinforced by continued comprehensive treatment & his progress was discussed regularly in the team. Gradually his physical condition improved, he looked brighter and more cheerful. He also started taking an interest in planning his future.

By mid-January, he gained bladder-bowel control, his bedsores healed and he started walking with a pair of callipers and elbow crutches.

His room in Kandivali was inconvenient for his present physical condition. Hence he decided to sell that & buy a room in Navi Mumbai where he decided to put a Chinese cart.

His case was discussed with the Social Worker of Sharan, Vashi who agreed to admit him for further rehabilitation.

At the time of writing this report the Social Worker learns that his mother visits him only twice a month. Until he starts his Chinese fast food cart , he is learning Tailoring & Computer to keep himself occupied.

 
This case serves to illustrate the importance of team–work and of counselling both for the patient and his family members, as well as the advantages of inter–agency co–operation.
